Clinical Pharmacist

Fantastic opportunity for a Practice based Pharmacist (IP Preference and interest in Diabetes)

The post holder will work as part of a multi-disciplinary team in a patient-facing role. The post holder will take responsibility for and undertake clinical medication reviews and support patients to proactively manage long term conditions.

The post holder will provide primary support to general practice staff with regards to prescription and medication queries. They will help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient in the GP practice.

If the post holder is not already a nonmedical prescriber, you will be supported to develop their role to become a nonmedical prescriber and develop an area of chronic disease management within the practice. The post holder will be required to enrol on the General Practice Pharmacist Training Pathway from CPPE (unless an equivalent qualification is already held, and exemption is agreed by CPPE).

Key Skills and Qualifications

  • To be courteous and polite to all people associated with the practice on the telephone, face to face or in written form.
  • To ensure confidentiality of information (written/oral or electronic) is always preserved whether at or away from work in accordance with the contract of employment.
  • To follow practice procedures to ensure that Caldicott Guardian and Security requirements are always met.
  • To follow procedures to ensure compliance with the Data Protection Act 1998
  • To follow all practice policies, protocols and procedures concerned including Safeguarding, with the maintenance of ethical practice.
  • To support and participate in initiatives to ensure a safe and healthy environment for all practice users. This includes following procedures to ensure the control of potential hazards to health and safety.
