This practice is seeking an enthusiastic Practice Clinical Pharmacist to deliver high-quality pharmacy-led services to our patients. This role involves working closely with our practice teams to improve prescribing practices, manage long-term conditions, and promote self-care. You'll lead medicines-related initiatives, mentor pharmacy staff, and drive operational efficiencies within the practice.
Key Responsibilities:
- Manage long-term conditions and perform structured medication reviews
- Provide telephone clinics and address prescription queries
- Lead prescribing audits and contribute to Quality and Outcomes Framework (QoF) initiatives
- Develop relationships with local community pharmacies and PCN pharmacy teams
- Deliver training for GPs and prescribers and promote safe prescribing practices
- Support care homes in improving medicines safety and management
Essential Requirements:
- GPhC registered pharmacist with a degree or equivalent in pharmacy
Desirable:
- Experience in primary care and clinical diploma
- Independent prescribing qualification
